INSTRUCTIONS FOR COMPLETING AND SUBMITTING YOUR APPLICATION
Print out Emblem Use Authorization Form.
- Fill out completely and mail to the address listed on the form. Remember to include your check or money order when sending in your Emblem Use Authorization Statement. You will not receive the authorized EUAS unless your payment has been received
- When MoSHA receives the form we will sign it and return it to you, along with DOR Form 1716.
- Bring both forms to the Dept. of Revenue when applying for your new plate.
You do not need to be up for renewal to get this plate. The DOR will charge you a small fee to transfer your current tags to the new plate.
