Oak Forest Dental Financial Planning

We would like our patients to be informed of our office financial policy. We are committed to providing you with the best possible care. If you have dental insurance we are eager to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our financial policy.

We accept cash, personal checks, MasterCard and Visa. In addition, we offer an excellent third party financial payment plan for balances over $1,000. Returned checks and outstanding balances older than 90 days may be subject to finance charges at the rate of 1.5% monthly. If you have dental insurance, you must bring proof of insurance and we will be more than happy to submit your insurance claims for you.
