Romance of The Three Kingdoms was written by Luo Guanzhong, which is based off of historical information and folktales revolving around the Warring States Period of China.
Maikuolan and Xiahou Dun are simply adding a spin to things.
Spoiler: The StoryThe setting begins in the last days of the corrupt Han Dynasty, showing how the government and Emperor lost the "Mandate of Heaven", and the land fell into anarchy, with various warlords carving out their own territories in a struggle for supremacy. Gradually, out of the chaos, three kingdoms take shape: the kingdom of Shu, led by the virtuous Liu Bei; the kingdom of Wei, led by the scheming Cao Cao; and the kingdom of Wu, led by the ambitious Sun family. All seek to unify the nation for one reason or another, and claim the right to rule for their own. And so the three kingdoms contend with one another over the century, and heroes rise and fall in the strife, until the nation is finally reunified.
Or, well, that's how it was supposed to go...
Roughly a year after the Shu kingdom established itself, history took a very odd turn. A kingdom to the west of the Han attacked and took control of the Hexi Corridor, and immediately interfered in a battle between Wei and Shu, sweeping in and taking control of the area, and continued its invasion further inland of Han. Finally, the invasion came to an end when Shu halted its assault in the midst of northern Jing.
When the dust settled, the invading country had almost completely separated Wei and Shu, and despite all attempts, neither could reclaim the lands from the stalwart defenses, and neither could the mysterious enemy take anymore land from the now aware countries.
Taking advantage of this pause, the countries, Shu, Wei and Wu, try a diplomatic approach to see just who it is their dealing with.
And this is where history marks the kingdom of Cha, lead by Xi Jian, and the fuel it added to the fires of chaos.
