Consent forms and Waivers

This DNP Project Implements a nutritional intervention in those who are overweight and obese.

If you are an adult,  age 18 and older who is overweight or obese this nutrition intervention may be for you!

This project will examine the effects of the implementation of a virtual nutritional intervention while striving to improve  BMI, weight, and health behaviors after participation in an 8-week virtual nutritional intervention program.

Participants will be asked to participate in

  • Self-reporting height and weight at the beginning and end of the study
  • Complete a pre- and post-9-question survey
  • Complete a 2 question pre and post Physical Activity Vital Sign questionnaire
  • 8 Weekly online module

This project seeks to implement a virtual nutritional intervention while improving weight, BMI, and health behaviors.
1. Shortened Life Expectancy

Obesity shortens life expectancy in individuals with moderate obesity (30 to 35 BMI) by three years.

2. Depression, Decreased Quality of Life

Adults with excess weight had a 55% higher risk of developing depression over their lifetime compared to people that did not.

3. Cardiovascular Disease

Excess weight can lead to build up in your arteries. If the arteries get damaged and clogged, it can lead to a heart attack.

4. Diabetes

Being overweight raises your risk for type 2 diabetes. It can also increase the risk of high blood pressure & cholesterol.

The Full Plate Living program highlights the benefits of whole food and a high-fiber diet without restricting food groups. The 8 week Full Plate Living program focuses on plate staging of 75% high-fiber foods, and the other 25% of the plate to consist of bread, steak, milk, and dessert.
